The accuracy of every measure in a facility depends on people scanning properly, reporting exceptions and reporting near-misses. All three stop when monitoring is read as adversarial.
The mechanism
Scan discipline degrades. People scan in ways that produce acceptable-looking data rather than accurate data.
Exceptions stop being reported. A problem reported becomes a problem attributed, so it is handled quietly and surfaces later, further downstream, more expensively.
Near-misses stop being reported, which removes the leading indicator that safety programmes depend on.
Workarounds go underground. The process appears to be followed and is not, so the recorded process and the actual process diverge.
Turnover rises, which costs more than the measurement saves.
None of this appears in the monitoring data, which continues to look fine.
What triggers it
Deployment without explanation.
Individual dashboards visible to supervisors.
The first disciplinary case built on monitoring data, which is observed by everyone and defines what the system is for.
Purpose drift, where safety data appears in a performance conversation.
Targets that arrive with the system, which tells people the measurement was the pretext.
Denial that it is monitoring, which insults people who can see what it is.
What avoids it
Say what it is, including that individual data exists where it does.
Consult before deploying, genuinely.
Fix something visible first. A monitoring programme whose first output is a fixed process problem is read entirely differently from one whose first output is a target.
Report at team level and mean it.
Write down what it will not be used for, and hold to it in the first hard case, which is when it is tested.
Give people their own data if they want it, which converts a surveillance object into a tool.
The first hard case
Someone will propose using the data for a disciplinary matter.
The decision made there is the policy, regardless of what the document says.
If the answer is yes, everyone learns what the system is for within a week, and the degradation described above begins.
If the answer is no, and the reason is stated, the assurance becomes credible in a way no policy document achieves.
Decide in advance who decides, because deciding under pressure produces the wrong answer.
Measuring the cooperation
Rarely measured and entirely measurable.
Scan anomaly rate, trended. A rise indicates discipline degrading.
Exception reporting rate. A fall is bad news presented as good news.
Near-miss reporting rate, where a fall is the clearest warning available.
Turnover, particularly among experienced staff.
Watch all four after any change to how monitoring data is used, which is when they move.
The honest position
A facility gets accurate data because people produce it accurately, and they do that when the system is not aimed at them.
The trade is real: individual measurement buys a small amount of insight and costs the accuracy of everything else.
Which is the operational argument for aggregation, independent of the legal one, and it is the argument that persuades people who are unmoved by the obligations.
The four cooperation measures
Rarely tracked and entirely trackable, and they move before anything else does.
Scan anomaly rate, where a rise indicates discipline degrading.
Exception reporting rate, where a fall is bad news presented as good news.
Near-miss reporting rate, the clearest available warning.
Turnover among experienced staff.
Watch all four after any change in how monitoring data is used, which is precisely when they move and precisely when nobody is looking at them.
Giving people the data first
A sequencing choice that changes how the whole programme is read.
Show the analysis to the people in the area before circulating it upward.
Ask whether it matches what they see, which improves the finding and settles the data quality dispute in advance.
Let them see their own figures before anyone else does.
Act on what they say, visibly.
A programme whose findings reach the floor last is understood as surveillance; one whose findings reach the floor first is understood as a tool, and the difference costs nothing.
